Porcine Lungs from the Slaughtering Process as a Model for Ex Vivo Lung Perfusion (EVLP)

Lung transplantation ist a safe and often lifesaving treatment for patients with end-stage pulmonary diseases. However, there is particulary great deficiency between transplantable lungs and potentially recipients. To adress this growing problem of organ shortages many centers already use marginal donor lungs (lungs which are sorted out before the transplantation because of demage) or extended-criteria heart-beating (HB) donors. Especially the ex vivo lung perfusion (EVLP) technique involves a great potential to extend the donor pool because lungs can be recondioning and treated while staying metabolic active. Furthermore the use of lungs from non-heart-beating (NHB) donors could be a possibility to deal with this problem of organ shortage. First preclinical studies suggest that an adaption of the ischemic times with the EVLP can also bring good results. However, questions remain regarding the optimal timing of EVLP and the best perfusion solutions. Instead of using lungs from organ harvesting in porcine models after euthanasia, we want to use lungs from the slaughter house. Isolated lungs of slaughtered animals should be compared with organs from euthanasied swine with regard to the respiratory parameters, parameters of the pulmonary vascular system and oxygenation capacity as well as histological and biochemical parameters.
